Purpose : Home Safety Self-Assessment Tool to :

  • increase perceived knowledge of home safety and identify unsafe activities,

  • develop home safety plans and safely perform activities,

  • reduce the risk of falls for elderly residents.


Admin time :  30 min. variable


User Friendly :  High


Administered by : Primary Carer, Nurse or Social Worker interview with individual and, where appropriate, their carer or someone else they nominate. May be used as a home self assessment by resident or carer..

  • The Home Safety Self Assessment Checklist and Solutions

  • Assistive Devices and Helpful Products to Prevent Falls

  • “How to” Home Improvement Instructions

  • Tips for fall prevention

  • Action Log
