The Vivifrail project is a program for the Promotion of Physical Exercise that is an international reference for community and hospital intervention for the prevention of frailty and falls in the elderly. It is currently being used by more than 5000 health professionals reaching an impact on the effective population of more than 15,000 people.
The Vivifrail project is part of the Strategy for the Promotion of Health and Quality of Life in the European Union.
Access to tailored programs (known as passports) for :
A. people with disabilities
B. people with fragility
B+ people with fragility and risk of falls
C. people with pre-frailty
C+ people with pre-fragility and risk of falls
D. robust people
E. robust people (Advanced level).
